Collision Report Information  

Washington State Law requires that all collisions involving injury, death or property damage in excess of $700.00 be reported within ten (10) days to the Department of Motor Vehicles.

In most cases law enforcement responds to these collisions and a police report/collision report has been completed and is forwarded to the Washington State Patrol and Department of Motor Vehicles. If this is the case you do not need to complete the self-reporting forms.

The below link from the Washington State Patrol website provides instruction, and forms for completing and submitting the State of Washington Vehicle Collision Report.

Q: What collisions require reporting?
A: The operator or owner of any vehicle involved in a collision within the State of Washington in which any person is injured, including one’s self, or in which any person’s property sustains damages in the amount of $700.00 or more is required to complete this collision report form if one was not completed by law enforcement. This includes out-of-state drivers operating their own or a rental vehicle.
   
Q: Can I report a Hit and Run collision with these forms?
A: Hit and Run collision (Damage to a vehicle, caused by another motor vehicle. Vandalism to a vehicle is not a Hit and Run) is a criminal offense. Hit and Run collisions must be reported immediately to the local law enforcement agency in which the Hit and Run occurred. The forms on the WSP link cannot be used to report a Hit and Run collision and you need to contact the jurisdiction in which this crime occurred.
